This notice describes how information about you may be used and disclosed and how you can get access to this information. Please review it carefully.

At San Diego General and Vascular Surgeons Medical Group, Inc., we are committed to treating and using protected health information about you responsibly. We reserve the right to change our practices and to make the new provisions effective for all protected health informtion we maintain. Should our information practices change, we will notify you at your next visit.

We will get your written permission before we use or share your protected health information for any reason other than treatment, payment, or health care operations, or as allowed or required by law. You may withdraw this permission at anytime, in writing. However, if we have already used your information based on your authorization, you cannot take back your agreement for those past situations.
